These are voluntary submissions, based on sheer enthusiasm, pride, public service commitment and a wish to show how clinicians in Primary Care Trusts are making a real difference. They provide inspiration, hope and practical examples to encourage clinicians and commissioners within every PCT to collaborate, innovate and improve services for their patients.
