Sharing the Learning from the Evercare Project

Sharing Resources

When considering how other PCTs could be successful without UHGs involvement – what resources have PCTs developed that would help other PCTs to implement Evercare and what other resources could be developed to help other PCTs implement this model.

 

What have PCTs developed that can be shared with other PCTs?

There were many resources developed at individual PCTs that could be shared with others through the NatPaCT website. These included:

  • Communication material to help with promotion and stakeholder engagement e.g. on the role of the nurse, patient leaflets, consent letters, Q&A, slides & presentations.

  • Sharing of work done to develop university training modules.

  • Project documentation e.g. Project Initiation Documents, Highlight Reports.

  • Clinical documentation such as standardised forms for APNs.

  • Model job descriptions/competencies – APN, champions, project management, data posts.

  • Data extraction tools such as queries to obtain data from different primary care information systems.

  • Supporting evidence for similar models such as report on ‘Social Care in Practice’ from Runcorn.

What could be developed for other PCTs?

  • Sound financial models to measure costs and benefits.

  • Computer/distance based learning materials for APNs.

  • The data that has been gathered at PCTs could be pulled together and shared more widely.

  • Further communications material could be developed to supplement those already out there.

  • HHhkeHelp to identify levers to change behaviours.

  • Help to identify and obtain the most important pieces of data.
