Towards Managed Care - Information Exchange Event
17th September 2004, Effingham Park Hotel, Copthorne

The purpose of this event was to expose a critical mass of key leaders across the system and investigating the range of approaches currently being adopted around the UK and exploring these topics together in more detail.

Stall 1: Evercare Mary Anne Smalley 
This programme has been developed in the USA to improve the quality of life for vulnerable older people by focusing on maintaining their independence, improving their wellbeing and avoiding the need to go to hospital.

Stall 2: Haringey TPCT/ Pfizer  Graham Prestwich 
The Haringey strategy for managing chronic disease incorporates a tiered approach, moving from those who can best manage their own care with their GP to those who have the most difficulty in managing their own health.  Haringey TPCT are working with DH and Pfizer Health Solutions to assess the feasibility and impact of an additional service for those people at the top tier.

Stall 3: Matrix Research & Consultancy Ltd  Mavis Williams 
The Matrix report 'Learning distillation of Chronic Disease Management programmes in the UK' presents the findings from a review, commissioned by the Modernisation Agency, comparing the different approaches to managing chronic diseases taken by United Healthcare, Kaiser Permanente and Pfizer Healthcare Solutions.

Stall 4: Serum  Perry Blatcher 
Following a successful national pilot in 25 PCTs, Serum, a leading independent provider of healthcare support and information, now offers a proactive telephone care management programme for patients with a number of long term conditions, enabling them to better self manage their condition, improve their quality of life and help avoid costly unplanned hospital admissions.

Stall 5: Kaiser Permanente  Dr. Hugh McIntyre 
America's largest not-for-profit health care organisation, Kaiser Permanente's aim is to minimise use of acute hospital beds through an integrated approach to service delivery.  It has a strong focus on the management of people with chronic diseases and the breaking down of barriers between secondary and primary care.

Stall 6: Telemedicine/ Modelling  Dr. Richard Curry 
Arising from a pilot project to rehabilitate elderly patients to avoid entering residential care, Dr. Curry and a team at Imperial College have developed a model to assess the impact of changes in health care provision on whole health and social care systems.

Doc@Home offers an integrated service for the collection, management and analysis of essential patient related data and permits efficient interaction between clinicians and patients at home.

The future and evolving role of NHS Direct (NHSD) in self care support and telephone advice in chronic disease management.

Stall 9: Managed Care in Mental Health 

Richard Ford 
Crisis resolution and assertive outreach teams are having far-reaching effects on mental health services across the country, according to a report published by the Sainsbury Centre for Mental Health (SCMH) and the National Institute for Mental Health in England (NIMHE).  Are there transferable lessons for managing chronic care?

Stall 10: Dartford, Gravesham & Swanley PCT  Gillie Lewis/ Janice Owen 
Modelled on the Evercare approach, Dartford, Gravesham & Swanley PCT redesigned the existing G grade District Nurse role to develop 9 new posts, each of whom carries a caseload of people with chronic care needs.  Their target is to reduce the amount of admissions to secondary care.

Stall 11: Western Sussex PCT  Jonathan Webster 
Western Sussex PCT are currently recruiting 5 Chronic Disease Case Managers.  These posts have been designed to provide advanced skills in helping people with chronic care problems to manage their problems at home and reduce unnecessary admissions to secondary care.
